Terms & Conditions
At Recovery IQ, we are committed to providing safe, professional, and high-quality care. These Terms and Conditions outline the expectations, responsibilities, and rights of both the client and the provider.
1. Eligibility for Services
Some of the wellness and recovery services that Recovery IQ offers are limited to clients aged 18 and older such as IV hydration and PRP injections.
Clients under the age of 18 may be eligible for certain services at the sole discretion of the provider, based on clinical appropriateness and safety. In such cases:
- A parent or legal guardian must be physically present at the minor’s first appointment.
- The parent/guardian must review and sign all required consent and intake forms before any services are rendered.
- Session parameters such as time, temperature, intensity, or modality may be adjusted for minors to ensure safety and age-appropriate care.
- Recovery IQ reserves the right to decline services to any minor if deemed inappropriate for their age, health status, or safety.
2. Right to Refuse Service
Recovery IQ reserves the right to refuse or discontinue services to any client for reasons including but not limited to:
- Disrespectful or inappropriate behavior toward staff
- Arriving under the influence of drugs or alcohol
- Non-compliance with policies or treatment recommendations
- Any condition that contraindicates safe treatment, as determined by the practitioner
3. Client Responsibility
Clients are expected to:
- Arrive on time for appointments
- Communicate openly and honestly with staff
- Follow aftercare or medical advice given by practitioners
- Provide accurate and complete health information
4. Payment Responsibility
- Clients are responsible to pay all fees for services they receive at Recovery IQ.
- Clients understand that payment is due at the time of service, unless a package or membership has been purchased in advance.
5. Card on File Authorization
By adding a card to their Recovery IQ Mindbody account, the client agrees to allow Recovery IQ to store their payment information securely and charge my card for services, no-show fees, late cancellations, packages, memberships, and any applicable taxes or fees.
6. Recurring Charges (if applicable)
- By enrolling in a membership with Recovery IQ, the client agrees that membership fees are billed monthly and will be automatically charged to the card on file.
- By enrolling in a membership with Recovery IQ, the client agrees to follow the terms outlined in the Membership Agreement, including cancellation and refund policies.
7. Missed Payments/Declines
- Clients are responsible for updating their payment method as needed to avoid interruption of services.
- By booking or authorizing payment for any services at Recovery IQ, the client agrees that if payment is declined, they may be subject to a $10 reprocessing fee and must resolve the balance before receiving further services.
8. Charge Disputes
By booking or authorizing payment for any services at Recovery IQ, the client agrees to first attempt to resolve any billing concerns directly with Recovery IQ before initiating a chargeback or dispute with any financial institution
9. Refund Policy
- Refunds will be issued at the sole discretion of Recovery IQ.
- Refund requests must be submitted in writing and are subject to review and approval by Recovery IQ, based on the circumstances of the request and the terms of the service agreement.
10. Limitation of Liability
While our practitioners are highly trained and services are delivered with care, individual results may vary. Recovery IQ is not liable for:
- Adverse reactions resulting from incomplete or inaccurate health disclosures
- Failure to follow post-treatment instructions
- Outcomes outside the scope of reasonable expectations

Recovery IQ’s Terms & Conditions are publicly available on our website at any time for client access and review. By using our services, clients acknowledge that they have had the opportunity to review these policies.
